[摘要]目的:通过分析安氏Ⅱ类1分类错牙合 治疗前、中、后软组织面型的改变,总结正畸治疗改善软组织面型的经验和方法,以指导临床工作。方法:选取安氏Ⅱ类1分类错牙合 患者26例,男10例,女16例,年龄10~26岁。于治疗前、中、后拍面像和X线头颅侧位片,参照Arnett 提出的软组织测量分析法进行分析测量,比较正畸治疗前、中、后软组织的变化。结果:软组织面型的改变主要在尖牙远中移动伴随切牙向舌侧调整的过程中(P﹤0.01),上唇突点距TVL后移2.09mm,下唇突点后移1.88mm,颏唇沟点后移0.91mm。治疗后微笑弧比率平均为男0.90,女1.22。结论:通过安氏Ⅱ类1分类错牙合 治疗前软组织面型分析,确定上颌前牙计划位置,并在治疗过程中根据软组织变化调整原来的计划切牙位置,可以获得满意的软组织面型。

Utilizing soft tissue analysis as a guide for orthodontic treatment for class Ⅱ malocclusion division 1

Abstract: Objective To study the principles and rules of class Ⅱ malocclusion subdivision 1 orthodontic treatment according to soft tissue analysis. Methods This study was carried out on a total of 26 subject,10 males and 16 females ranging in age from 10 to 26 years. Each patient was taken pictures and standardized lateral cephalograms before and during and after the treatment. Totally 5 soft tissue landmarks were evaluated in horizontal directions according Arnett's soft tissue cephalometric analysis. Results Soft tissue faces changed largely when canines were pulled distally. UL-TVL reduced 2.09㎜, LL-TVL reduced 1.88㎜, B′-TVL reduced 0.91㎜.Average smile arc ratios are 0.90 and 1.22 in males and females respectively. Conclusion Patients with class Ⅱ malocclusion subdivision 1 can get pleasing soft tissue faces and esthetic smile when planning maxillary incisor′s position was established according soft tissue facial analysis.
