[摘要]目的:观察改良钟摆式矫治器在远移上颌磨牙方面的作用及效果。方法:挑选23名患者,其中男10名、女13名,平均年龄12.6岁。患者均为牙源性安氏Ⅱ类错牙合 ,恒牙列且下颌牙弓排列较好。改良钟摆式矫治器是新近研制的一种远移上颌磨牙的装置,它焊接于两侧第一磨牙和第一前磨牙的带环上,其支抗是腭侧的Nance腭托。本研究采用150g力的Ni-Ti螺旋推簧远移上颌第一磨牙,于治疗前与磨牙达到安氏Ⅰ类关系后用头影测量分析测量上颌磨牙的远移效果。结果:上颌磨牙平均整体远移5.1mm,平均远移时间为3~6个月,前牙覆牙合 减少2.9mm,覆盖增加3.4mm。远移完成后,为防止远中移动的上颌磨牙向近中移动而造成复发,用Nance腭托将纠正后的安氏Ⅰ类牙合 关系保持3个月。结论:改良钟摆式矫治器可以在较少的支抗丧失情况下有效的远中整体移动上颌磨牙。

The clinical research of modified pendulum application for upper molar distalization

Abstract: Objective To evaluate the clinical effects of modified Pendulum appliance on upper molar distalization in classⅡ cases. Methods Twenty-three patients ,which include ten male and thirteen female with mean age of 12.6 years, were selected for molar distalization. All presented Class Ⅱ molar relationship. The subjects were all with a well aligned lower dental arch. Modified Pendulum appliance is a new intra-oral appliance for maxillary molar distalizalion ,which was composed of two premolar bands and two molar bands. The anchorage unit was a Nance button. A Ni-Ti coil spring was used and 150g distal force was applied to each upper first molar. Cephalometic ridiographs were taken and analyzed before and after molar distalization. Results The upper molars were distalized bodily on average 5.1mm in 3-6months. The overbite was reduced 2.9 mm and the overjet was increased 3.4 mm. For stabilization, the corrected molar relationship was maintained with a Nance button for 3 months. Conclusion The Modified Pendulum appliancs can achieve bodily distal molar movement with minimum anchorage loss.
